A standard course evaluation questionnaire is administered in all classes during the last two weeks of the term. End-of-term feedback is important but I also want your input while you are still in the course.

You may use this form to send me anonymous feedback about the course. Feel free to make any comments or suggestions that you would like to express. Some questions in which I am particularly interested include:

• What do you like about the course?
• What would you change about the course?
• How could the course Web site be improved?
• What could I do to enhance your learning of the material covered?
